The Problem: Physician Burnout & Lack of Real-Time Insight
Physician stress and burnout have reached critical levels, with 45-75% of physicians reporting burnout symptoms.
Burnout is associated with:
- Emotional exhaustion and reduced quality of care
- Increased medical errors and lower patient satisfaction
- Higher physician turnover, with replacement costs up to 2-3x annual salary
- Significant system-level costs, estimated at $4 billion annually in the U.S.
Despite the scale of the problem, current approaches to measuring burnout are limited:
- Infrequent surveys that miss fluctuations in stress
- Low response rates (often 40-50% or lower)
- High reliance on recall, reducing accuracy
The challenge is capturing real-time, continuous insight into physician well-being to enable timely and targeted intervention.
What is CalmDoc?
CalmDoc is a text-based stress monitoring system designed to continuously assess physician well-being in real-world clinical environments.
Using a validated single-item stress question (SISQ) delivered via SMS, CalmDoc captures real-time stress data across individuals and institutions. The SISQ has been shown to correlate with key indicators of physician well-being, including burnout, depression, and intent to leave.
Participants receive:
- One brief stress check-in per week, sent at randomized times
- A simple response format (e.g., replying with a number)
- Ongoing engagement with minimal burden
At the system level, CalmDoc enables:
- Continuous, coordinated stress monitoring across institutions
- Identification of temporal trends (when stress peaks)
- Detection of organizational “hot spots” (where stress is highest)
CalmDoc leverages principles of Ecological Momentary Assessment (EMA) to capture real-time experiences, reducing recall bias and improving the accuracy of stress measurement.
The platform also includes:
- Personal and institutional dashboards displaying stress trends
- Anonymous feedback mechanisms to support engagement and transparency
CalmDoc provides a simple, scalable approach to understanding and responding to physician burnout.

Outcomes
CalmDoc has demonstrated:
- High engagement and response rates
- SMS-based assessments can achieve response rates of 90-98%, significantly higher than traditional surveys
- Improved accuracy of stress measurement
- Real-time assessments reduce recall bias and better reflect lived experience
- Actionable insights at both individual and system levels
- Enabling identification of stress patterns across large clinician populations
- Enhanced ability to target interventions
- Continuous data allows institutions to identify when and where support is most needed
